MYHA Registration Information
2010-2011 MYHA Registration
Existing Members
If you were not able to make it to the posted registration dates of August 2nd and 3rd, or cannot make the new member registration evenings of the 13th and 14th of September, you are more than welcome to come in
to the office, any time after August 3rd to register between the hours of 8 am - 1 pm, Monday through Thursday. Beginning September 6, office hours will be 8 am - 4 pm, Monday through Friday. Please check out the list below of what to bring.
A late fee of $50 will be assessed to "each current player" that registers after 12:00 Noon on September 15, 2010. This fee does not apply to new players or to Jr. Gold players. ALL over due fees and/or unpaid fundraising fees from the prior season must be paid prior to registering for the 2010-2011 season.
New Members
Registration for new members will take place the 13th and 14th of September, 6 - 8 PM on the turf in the North Rink. Please check out the list below for what to bring.
What To Bring
*All completed registration forms, pdf's available below.
*USA Hockey confirmation page w/ barcode - ALL skaters must register regardless of age. Skaters age 7 and over during the 2010 season will be accessed the $40 fee.
*Copy of State/Government issued Birth Certificate (new members)
*Check and/or cash - mandatory payments for fundraising, volunteer hours and Tobolt Lease Equipment Program.
*Check, cash or credit cards are accepted for membership, level fees, & travel fees.
