MYHA Registration Information
2011-2012 MYHA Registration
A late fee of $50 will be assessed to "each current player" that registers after 12:00 Noon on September 15, 2011. This fee does not apply to new players or to Jr. Gold players. ALL over due fees and/or unpaid fundraising fees from the prior season must be paid prior to registering for the 2011-2012 season.
What To Bring
*All completed registration forms, including signatures
*USA Hockey confirmation page w/ barcode - ALL skaters must register regardless of age. Skaters age 7 and over during the 2011 season will be accessed the $40 fee.
*Copy of State/Government issued Birth Certificate (new members)
*Check and/or cash - mandatory payments for fundraising, volunteer hours and Tobolt Lease Equipment Program.
*Check, cash or credit cards are accepted for membership, level fees, & travel fees.
